Hi, I'm Cheryl, a Credentialed Diabetes Educator (CDE) and Registered Nurse.

As a Credentialed Diabetes Educator (CDE) working within a general practice setting in the Hawkesbury region, I play a central role in supporting patients to prevent, understand, and self-manage diabetes across the lifespan.My work sits at the intersection of clinical care, patient education, behaviour change support, and multidisciplinary teamwork.

Key Areas of Expertise

  • Comprehensive Diabetes Education:
    Providing individualised education on type 1, type 2, gestational diabetes, and pre-diabetes, with clear explanations that empower patients to manage their condition confidently.

  • Self-Management Support:
    Coaching patients in blood glucose monitoring, insulin administration, medication adherence, recognising hypo/hyperglycaemia, and lifestyle adjustments (nutrition, physical activity, stress, sleep).

  • Care Coordination in General Practice:
    Collaborating closely with GPs, practice nurses, dietitians, podiatrists, and other allied health professionals to deliver seamless, patient-centred care within the Medical Home/Chronic Disease Management framework.

  • Technology-Enabled Diabetes Care:
    Supporting patients with CGM, insulin pumps, digital monitoring tools, and telehealth follow-up.
